Opt-out of the RECOVER IV study
Anyone from the community who does not want to be enrolled in the study can choose to opt out ahead of time. Opting out of the study involves wearing a wrist band with the inscription “No RECOVER IV Study” during the duration of the 5-year study.
To request an opt-out wristband please fill out the contact form below. If you prefer, you are welcome to call the toll-free number below to request an opt-out wristband. You will receive the silicone “No RECOVER IV Study” wristband by US mail.
By wearing this wristband, you inform medical personnel conducting this research that you do not want to participate in this study.
